IMPORTANT INFORMATION
The Social Security Administration has to require you to have a representative payee if needed. If you have not been required to have a representative payee, please contact Social Security before filling out any of the following forms.
If you are required by SSA to have a rep payee, please note we can only accept clients that live within 75 miles of our office (Walpole, MA) - please make sure before applying that you fall within that radius.

To apply for our services, please print and complete all forms which can be found linked at the bottom of this page.

Additional paperwork may be needed:
If the client is currently receiving their own disability check, they need have their physician/psychiatrist complete the SSA-787 form which can be found online/in most doctors offices and submit the completed form with their application or directly to SS in order for us to apply.
If this person has a legal guardian, we would need a copy of the guardianship papers as well as a signed letter from their guardian stating they do not want to be payee for this person.
IF WE DO NOT RECEIVE ALL REQUIRED FORMS FULLY COMPLETED, THE APPLICATION PROCESS WILL BE DELAYED. PLEASE DO NOT SEND COPIES OR TRY TO FAX/EMAIL THE APPLICATION DOCUMENTS. WE CAN ONLY ACCEPT THE ORIGINALS BY MAIL.
